Office Policies

In order to stay on time and provide the most comprehensive care for our patients, we require that all patients call to make an appointment. Please call (402) 391-3010 for our Omaha office and (402) 332-3903 for our Gretna office to schedule an appointment. Our appointment line is open from 8:00 a.m. - 5:00 p.m. Monday through Friday and 8:00 a.m. – 12 p.m. on Saturdays.

Please be on time for your appointment. We request that you arrive 5 to10 minutes early for your appointment to allow time for check-in. If you are late for a preventative healthcare visit, we may need to reschedule you, so that our providers can stay on time for all patients. Our providers strive to stay on time during the day but sometimes emergencies or complicated visits can happen. We thank you for your patience during these times.

We request that you give us 24-hours notice if you are unable to keep an appointment, so that we may open it to another patient.

No Show Policy is as follows:

  • 1st NO SHOW / LATE CANCELLATION patient reminded of policy.
  • 2nd NO SHOW / LATE CANCELLATION patient charged $30.00. (Penalties must be paid before the patient can be seen!)
  • 3rd NO SHOW / LATE CANCELLATION patient charged an office visit. (Penalties must be paid before the patient can schedule an appointment!)
  • 4th NO SHOW / LATE CANCELLATION patient is dismissed from the practice.

NO SHOW is any missed appointment, or any appointment that isn’t cancelled 24 hours in advance. This policy applies to a I2-month period and not a calendar year. Penalties must be paid before the patient can be seen.

Please bring your insurance card and driver license and your required co-payment to each visit

Office Protocol

In order to keep our office clean, food and drinks are not allowed in our reception area or exam rooms.

We need your full attention at your visit and request that you silence your cell phones in our office.

Message and Call Back Policy

We strive to provide the best in customer care and want any questions or concerns to be attended to in a timely manner. Our message and call back policy is designed to respond appropriately to phone messages while allowing our providers to give the focused care patients need during office visit. Please allow 24hrs to 48hrs turnaround time for call backs. Our providers work closely with our nurses and encourage patients to contact our nurse advice line first for any health-related issues

Prescription Refills

Please direct prescription refills through your pharmacy, as they are transmitted electronically and have all the needed information available.

Lab and X-Ray Results

We will call you with lab and x-ray results as soon as possible. If we have been unable to reach you or if you have any questions, please feel welcome to call our offices at (402) 391-3010 for Omaha and (402) 332-3903 for Gretna.

Medical Records and Forms

We complete most forms at no charge to our patients. Please note that most sports or camp forms require an up-to-date well child exam. You may leave the forms at any of our offices for completion, and we will contact you when the form is ready to be picked up. Turnaround time is usually between 24hrs to 48hrs.

  • Transfer or copying of medical records requires a signed release form from the parent or the patient (if over 18). We will send one set of records per patient free of charge to another physician.

Southwest Family Physicians & Gretna Family Health fully complies with all HIPPA regulations and our privacy policy had been made available to all patients.
